Concierge General Information

  1. Appointments & Doctor’s Visits: We recommend an appointment every 1-3 months, sooner if needed. We strive to schedule visits in advance. To book an appointment, text/call our office Monday through Friday, between 9:00 AM and 5:00 PM. 
  2. Insurance: House-call visits are covered by insurance part B and most secondary insurance plans. PPO/POS insurance plans often have out-of-network benefits.  Contact your insurance directly to verify benefits, copay, deductible or related questions.
  3. Coordination of care: Your PCP coordinates Office Hours: Our staff is available Monday through Friday, 9:00 AM to 5:00 PM, to assist with texts/calls. Prescription refills and other requests can also be faxed by the pharmacy to our office.
  4. After-Hours Assistance: evenings, weekends and holidays you can reach a clinician by calling/texting the main office number below.  For routine questions, contact us during office hours. 
  5. Urgent Visits: While we often accommodate medical appointments within 1-2 business days, we are not a substitute for the ER or urgent care. In case of emergencies, please call 911 immediately. Do not wait for instructions from your physician.
  6. Narcotics and Other Controlled Medications:  We do not prescribe opiates or controlled substances over the phone.  We do not carry narcotics or controlled substances with us. 
  7. Home Health & Referrals: We can arrange home health evaluation to address needs like wound care, and assessment for physical, occupational, or speech therapy. We also coordinate with specialists such as hospice, podiatrists, dentists, orthopedists, cardiologists, psychiatrists, and others. Please note: most specialists do not provide house calls.
  8. Diagnostic Studies: Through our local radiology partners, we can refer for  house-call diagnostic services, including EKGs, X-rays, and ultrasounds.
  9. Vaccines: During regular business hours we can arrange routine vaccines covered by Medicare Part B: Influenza, RSV, COVID-19, and Pneumonia. Vaccines not covered by insurance may incur an out-of-pocket cost and be billed by the pharmacy.  Advance coordination is needed.
  10. Medication Delivery (optional): Monthly prescription refills can be delivered to your home during regular business hours through an independent community pharmacy.  A delivery fee may apply.
  11. Ancillary Services: When scheduled in advance, we perform routine bedside procedures such as ear lavage for ear wax removal. To schedule, contact our office with specific requests or questions about tests or procedures.
  12. Blood work: Mobile in-home blood work and labs are able to be scheduled through mobile labs and they have a travel fee that is billed directly by them to you. We send lab orders directly to the lab.
  13. Reports & Letters: Physician’s Reports and TB clearance documents required for Assisted Living or Board & Care facilities can be completed within 48 hours. There is a standard fee (waived for concierge patients) for letters, medical documents, and RCFE forms. Extensive medical documentation may incur additional charges. Contact us for more information.
  14. Finances, Accounting and Billing: Direct your accounting or billing related questions to the office. Our mobile staff and clinicians do not carry or accept any money/cash.

    *NOTE: Urgent, after-hours, weekend, and holiday services for vaccinations, labs, imaging, reports, or medication delivery may incur additional charges from the pharmacy, lab, radiology, provider directly. Insurance co-pays and deductibles apply to all appointments and covered services.

Triage, Appointments and Referrals

We help triage your concern to determine which course of action may be recommended for you.  For example, symptoms such as for minor symptoms may usually be evaluated and work up can be initiated at home. There may be symptoms for which we need to refer you to urgent care or the emergency room. 

Some conditions and symptoms such as chest pain, shortness of breath, severe abdominal pain, intractable vomiting, high fever, dehydration, lethargy or change in mental status, sudden weakness are a few examples of life threatening conditions which may need an evaluation in the Emergency Room or need to call 911.
