When a loved one is moving into an Assisted Living Facility, Board and Care, Memory Care or other Living Community, California requires the 602A Physician’s Report. This form documents medical history, diagnoses, medications, functional abilities, and special care needs. It helps facilities ensure your loved one receives safe, appropriate care. We provide telehealth (virtual video) appointments with a licensed physician who can complete the 602A form after meeting the patient and reviewing their medical history.

What We Need From You Before the Appointment:

  • Copy of front and back of insurance cards and ID
  • Download and send in completed Intake Forms (sent via secure email or 🔒 Secure App: Spruce)
  • The 602A Form provided by the facility
  • Copy of Health POA or Advance Directive (if applicable)
  • Recent Medical Records (available through your previous primary care or your health portal)
  • Recent Vitals – Blood pressure and heart rate (can be taken at home if available)
  • TB Screening Results – A PPD or chest X-ray (required for facility admission)

    Download and Fill out the Telehealth Intake Form and send us the facility’s The 602A Form along with any supporting documents.

3. Telehealth Appointment
Meet with our physician via Zoom. Your loved one should be present, even briefly, so the doctor can complete the exam portion.

4. Form Completion
The 602A will be filled out during the visit. If TB test or chest X-ray results are pending, the form can be finalized once results are available.

5. Delivery
Completed forms are securely sent back via email or the Spruce app.
